First Visit

Your initial appointment usually will consist of dental examination and review of your treatment options. In some emergent cases, treatment can be done or started the same day as the consultation. However, a routine initial visit will involve a obtaining full set of x-rays, a dental cleaning, and a comprehensive examination.

Please assist us by providing the following information at the time of your first visit:

• Any x-rays taken by your local dentist, if applicable. In most cases, we will take our own x-rays at your first visit.
• A list of medications you are presently taking.
• If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert our dental office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

Dr. Adam Shoukry, DDS, is a cosmetic dentist who strives to provide each of his patients with personalized care and educate them on the state of their dental health so that they can make informed decisions. Dr. Shoukry really enjoys his work and is committed to continued education, having attended several accredited courses in every aspect of dentistry.

Dr. Shoukry graduated from NYU College of Dentistry with a Doctor of Dental Surgery degree and then completed his general practice residency at Nassau University Medical Center. Being a member of the American Dental Association, the Academy of General Dentistry, and the American Academy of Cosmetic Dentistry, Dr. Shoukry strives to provide comprehensive dental examinations, a full range of cosmetic treatments, and the best restorative oral health care available.
